It is with great sadness that NC Nuropsychiatry is closing their Raleigh and Apex locations; however, I will continue seeing patients through my private practice, here at Carolina Cognitive and Behavioral Health.
To avoid interruptions in your treatment, I encourage you to arrange for continued healthcare services as soon as possible whether it is through me or another practice.
If you wish to continue care with me, you may request to establish care here by completing the form linked below. You have the option to see me in-person and/or telemed. Telemed only is also an option so long as it meets current practice laws.
Using the link below, you can express your preference to remain within my care and request the transfer of your medical records from NCNP to my private practice. Please know your decision is entirely voluntary, and you are free to choose any qualified provider/practice. Selecting Carolina Cognitive and Behavioral Health is not required to receive your records from NCNP.
Should you opt-in, my practice will make every effort to ensure an easy transition and reduce the work and inconvenience on your end. Upon completion, someone at Carolina Cognitive and Behavioral Health will contact you regarding your next steps and appointment date shortly.
