Appointments
We understand that life happens. If you are unable to keep an appointment, please notify our office as soon as possible.

Appointments missed or cancelled without at least 24 hours’ notice will incur a $50 fee.

Please arrive on time for appointments. Arrivals 10 minutes or more late may need to be rescheduled to ensure all patients receive the full appointment time.
Medical Records
We are happy to provide copies of your medical records.

Please allow 7 business days for processing.
Forms & Letters
Completion of forms, letters, or other documentation requires review of your chart.

Please allow 5 business days.
Medication Refills
To ensure continuity of care, please submit refill requests by calling or texting 984-399-9688 at least 5 business days in advance.
Prior Authorization Requests
If your medication or treatment requires prior authorization through your insurance provider:

We strive to receive approval same day or as soon as possible; however, responses are determined by the insurance company. Please allow 3-5 business days for processing.

Some requests may take additional time depending on your insurance carrier.
Insurance & Payment

Patients are responsible for co-pays, deductibles, and any charges not covered by insurance at the time of service.

We accept payment via cash, credit/debit card, and major insurance plans (please confirm coverage prior to appointment).
Telehealth

Telehealth appointments are available.

Please ensure a private, quiet location for your session and a secure internet connection.
Communication

Routine messages may take 3 business days for a response.

For urgent issues after hours, you can leave a voicemail and your call will be returned within 1 business day.

For emergencies, please call 911/988 (suicide hotline) or go to your nearest emergency department.
Prescription Policy

Unless previously discussed during our appointment, new prescriptions or dosage adjustments require an in-person or telehealth evaluation. This is to ensure necessity and for your safety.

Controlled substances may have additional state and federal restrictions.
Confidentiality & Privacy

All patient information is kept confidential in accordance with HIPAA regulations.

Information may only be shared with your consent or as required by law. We are happy to send records or discuss your care, but please understand a release must be completed prior.
Emergencies

If you are experiencing a psychiatric or medical emergency, call 911 or go to your nearest emergency department immediately.

This office does not provide emergency care outside of scheduled appointments.
